How to Submit Your Appeal Request Form

 

You can appeal the decision if Pennie said you were not:

  • Eligible to buy a health insurance plan.
  • Eligible to buy Catastrophic coverage.
  • Allowed to enroll or change your health plan with a Special Enrollment Period.
  • Able to get an exemption.
  • Eligible for financial savings to help with health coverage costs.
  • Eligible for the amount of financial savings you thought you were eligible to receive.

 

Note: If Pennie is requesting you send us documents please do not file an appeal.  You can only appeal final eligibility decisions.  You have 90 days from the date of your final eligibility determination to file the appeal request.

 

 

Instructions:
    1. Download and complete Pennie’s Appeals Request Form or en Espanol.
    2. Attach supporting documentation.
    3. Return the completed form to Pennie.

Note: You can add someone to file the appeal and speak with Pennie on your behalf on pages 11 and 12 of the Appeal Request Form.

You may request a faster appeal if you have health risks.

All appeals from Pennie actions are governed by 45 C.F.R. §§ 155.500-155.555 and the General Rules of Administrative Practice and Procedure, 1 Pa. Code Part II, Chapters 31-35.

 

 

If this is your first attempt to reach Pennie, you can file a complaint.  This may be a faster, easier way to get help.
      1. Call Pennie at 844-844-8040 to request a compaint ticket be created (or)
      2. Log into your Pennie account.
        1. From your Pennie Dashboard, select “My Tickets”.
        2. Create a new ticket.
        3. Add supporting documents and click “Submit”.
        4. Follow-up or add comments to your ticket anytime via “Ticket History”.

Please allow 30 days for Pennie to review your complaint and contact you.
